El objetivo de esta revisión es sintetizar la evidencia actual y las recomendaciones prácticas sobre el manejo de las urgencias y los cuidados intensivos en reptiles, ofreciendo a los clínicos un marco actualizado para su estabilización y tratamiento. La revisión aborda nueve áreas clave: anamnesis y triaje; manejo y condiciones de mantenimiento; examen físico; estabilización y reanimación cardiopulmonar (RCP); monitorización; fluidoterapia; transfusión sanguínea; analgesia y tratamiento de soporte; y diagnóstico. Se destacan las características fisiológicas que diferencian a los reptiles de los mamíferos, y cómo estas adaptaciones condicionan la fluidoterapia y la estabilización en situaciones de urgencia. Se enfatiza la rehidratación gradual y controlada dentro de la zona de temperatura óptima preferida (POTZ), así como las indicaciones específicas de las vías intravenosa, intraósea y subcutánea. Asimismo, se revisa la evidencia disponible sobre el uso de analgésicos, en particular antiinflamatorios no esteroideos (AINEs) y opioides, destacando la administración intramuscular de ketoprofeno y el predominio de receptores μ y δ. Se analizan las principales pruebas diagnósticas, como las analíticas sanguíneas, las técnicas de imagen y los cultivos en el contexto de la termorregulación. En el futuro, las investigaciones deben centrarse en ampliar los estudios farmacológicos, validar las pautas analgésicas y antibióticas y estandarizar los protocolos de monitorización y triaje para mejorar los resultados obtenidos en los cuidados intensivos en reptiles.
Abstract
The objective of this review is to summarize current evidence and practical approaches in the management of reptile emergencies and critical care, offering clinicians an evidence-based framework for stabilization and treatment. The review focuses on nine key areas: history and triage, husbandry assessment, physical examination, stabilization and cardiopulmonary resuscitation (CPR), monitoring, fluid therapy, blood transfusion, analgesia and supportive care, and diagnostic testing. The physiological characteristics that distinguish reptiles from mammals are highlighted, along with their influence on emergency stabilization and fluid therapy. Controlled, gradual rehydration within the preferred optimal temperature zone (POTZ) is emphasized, along with specific indications for intravenous, intraosseous, and subcutaneous routes. Evidence for analgesic use, particularly non-steroidal anti-inflammatory drugs (NSAIDs) and opioids, is reviewed, with intramuscular ketoprofen appearing to be a suitable option based on current evidence and with μ- and δ-receptor predominance explaining interspecies variation in opioid response. Diagnostic priorities, including blood analysis, imaging, and culture techniques, are discussed in the context of thermoregulation and stabilization. Future research should focus on expanding pharmacological studies, validating analgesic and antimicrobial regimens, and standardizing monitoring and triage protocols to improve outcomes in reptile critical care.

El objetivo de este artículo es revisar de forma práctica el abordaje inicial, la estabilización y los cuidados intensivos en reptiles, integrando aspectos clave como el triaje, la evaluación del manejo, la fluidoterapia, la analgesia, la monitorización y las principales pruebas diagnósticas.
Anamnesis y triaje
El triaje en reptiles comienza en el momento en que el propietario contacta con la clínica. El personal de recepción y auxiliar debe estar formado para distinguir las verdaderas urgencias de aquellas situaciones que pueden esperar una cita programada sin riesgo para el paciente. Conclusión
El manejo de las urgencias y los cuidados intensivos en reptiles requiere un abordaje específico para cada especie, en el que se prioricen la corrección de los factores ambientales, la reducción del estrés y la estabilización fisiológica previa a las pruebas diagnósticas. La comprensión de las adaptaciones metabólicas y cardiovasculares únicas de los reptiles permite realizar una reanimación y recuperación eficaces, generalmente con intervenciones menos agresivas que en mamíferos. El triaje adecuado, una evaluación detallada de las condiciones de mantenimiento y una monitorización cuidadosa siguen siendo los pilares fundamentales del éxito en la medicina de urgencias en reptiles.
